Home Health Team

The following are just a few of the examples of services that are available under our Home Health program:

Nursing
A Registered Nurse visits a client in their home to monitor their medical condition and assist with special health care needs.

On the initial visit, the nurse discusses a plan of care with the client to facilitate a return to independence.

Physical Therapy
The Physical Therapist develops a home exercise program based on the client's medical needs.

The goal of treatment is to facilitate a return to independence with activities of daily living.

Occupational Therapy
The Occupational Therapist assesses the client's home and makes suggestions for modifications to help the individual function independently in the home.

Speech Therapy
The Speech Therapist develops a plan of care to assist with speech, swallowing, non-oral communication and language difficulties.

Medical Social Services
A Social Worker assists the client with long-term planning and identifies community resources available to meet the client's needs.

Also assists in identifying social factors impeding recovery and coping mechanisms to assist in rehabilitation.

Nutrition Consultation
A Registered Dietician provides assessment and guidance with regard to nutritional requirements.

Certified Home Health Aide
A Certified Home Health Aide assists clients with personal care, home exercises and activities of daily living.

Volunteers
Our Volunteers are trained to provide respite for family and/or caregivers, transportation, errands assistance and visitation and companionship.
