Who is eligible for your services?
For Home Health and Hospice services, your primary care physician will order the care. However, anyone can make a referral to Homecare & Hospice for services at any time. For example, your spouse may call to inquire about services and then we would gladly contact your doctor to discuss orders.

Home Health and in-home Hospice care is for individuals living in Manhattan (including assisted living centers or nursing homes) and a 30 mile surrounding radius. Hospice clients cared for at The Good Shepherd Hospice House do not have to live within that service area as the hospice house will accept clients residing in any county or any state.

Health Care Attendant services do not require a doctor’s order and anyone can refer to services. We provide Health Care Attendant services to individuals living in Manhattan and a 30 mile surrounding radius.
